Young Carers Card

The young carer’s card is for young carers who are having problems at school because they are looking after someone at home.

Companion Card

A Companion Card lets you take someone with you free of charge when you travel on buses in Tyne and Wear, County Durham, Northumberland and Darlington. It’s available to residents of Tyne and Wear who receive certain allowances and need a companion to make all, or some, journeys.

Carers Passport

South Tyneside and Sunderland NHS Foundation Trust recognises that carers play an important role in the continuing care of many of their patients during their hospital stay and in planning their discharge. Their Carer Passport Scheme helps to identify you as a carer.

Carers Assessments

If you care for someone, you can have an assessment to see what might help make your life easier. This is called a carer’s assessment.

GP Carers Register

Tell your GP that you have caring responsibilities as soon as possible,
they can then record this on your medical records.

If they know you are a carer and likely to be under pressure at times, they will be able to offer more tailored advice and, if necessary, provide more support when they diagnose and treat you in the future.
