What is Social Prescribing?
Many things can affect your health and wellbeing. We know that it can take more than medicine to help you get better.
From time to time we all need support with making lifestyle changes that can improve our health and quality of life.
Social Prescribers will work with you to find out ‘What matters to you’. The team will then support you in your own community.
What does a social prescriber do?

Many of life’s problems can make you feel unwell. We can help you with a range of things like:
- Loneliness
- Emotional Wellbeing
- Money worries
- Housing worries

How do I access the service?
Complete the referral form below or ask your GP to refer you to the Sunderland Social Prescribing Service
If you, or someone you know, is experiencing a mental health crisis, you can now call NHS 111 and select option 2 for 24/7 access to crisis mental health support.
The service is free and available to anyone registered with a GP in Sunderland.
