Blue Badge
The Blue Car Badge scheme helps people with disabilities access essential community facilities.
Dementia Support Line
The Alzheimer’s Society provide a Dementia Support Line.
If you’re affected by dementia or worried about a diagnosis, or if you care for someone with dementia, their trained staff are ready to give you the support that you need.
District Nurse
The District Nurse Team provide holistic nursing care to housebound patients preventing unnecessary hospital admission and allowing patients to be cared for in their own home or chosen environment.
Essence Service
The Essence Service provide a free and dedicated service for people who have been diagnosed with mild to moderate dementia and their carers, as well as a pre-diagnostic support for those who may be on the pathway to a diagnosis.
Fire Home Safety Check
Tyne and Wear Fire and Rescue Service offer free “Safe and Well” visits to assess fire risks in your home and provide advice and potentially install smoke alarms. There is also a home fire safety check you can complete yourself.
NHS Social Care and Support Guide
The NHS have created a Social Care and Support Guide, if you or someone you know needs help with day-to-day living because of illness or disability, the guide explains your options and where you can get support.

Recovery at Home
Recovery at Home operates 24 hours a day, seven days a week, ready to respond quickly to provide support during times of illness or if someone experiences an unexpected change in their condition that could develop into a health crisis.
Specsavers Home Eye Test
If you, or someone you know, are unable to leave home unaccompanied due to a physical or mental illness or disability, Specsavers can come to you and perform a home eye test.
Telecare
Sunderland Telecare system provides a wide range of Telecare equipment that is linked to Sunderland City Council’s highly trained call handlers who will assist you if you are in difficulty and deploy a mobile response worker to your home if required.
